A new beginning, built around the member

Helping people return to community life with confidence.

Our Transition Coordination Management services support individuals who want to move from a nursing facility into a home or community-based setting. We work with the member, their care team, case manager, family, and community partners to remove barriers and keep the transition moving forward.

Each plan is tailored to the member. Services are coordinated with the member’s approved benefits, care team, and transition goals.
